The Forms Editor now supports deep integration with addy Business fields and Data Room files. These Forms will be presented as critical Checklists for Issuer and addy Staff.
The Forms Editor now supports deep integration with addy Business, allowing custom forms to directly interact with addy Business fields and Data Room files. This integration enables dynamic checklists that adapt based on the properties of issuances.

- At the bottom of the Forms Editor, click the "+ Add addy Business Field" button
- In the modal dialog, you'll see a tree selector showing available addy Business fields and Data Room locations
- You can either:
- Navigate the tree structure to find your field
- Start typing to filter and quickly locate fields
- Select your desired field and click "Add"
Note: When you add an addy Business field, the form automatically:
- Sets the appropriate data type
- Grants Read/Write permissions to the "Issuer" role
- Converts the form into a Checklist-only format
You can make form fields appear only when specific conditions in addy Business are met:
- Click "Advanced Options" on any field
- Select "It depends on something in addy Business"
- Choose the addy Business field to depend on
- Enter the value(s) that should trigger this field to display
- Click "Save" to confirm the conditions
After saving your form with addy Business fields:
- Click the "Preview Checklist" button in the upper right
- Use the Entity autocomplete to select a specific entity
- Click "Preview Checklist" to see how the checklist will appear for that entity
Important: Always save your form before previewing the checklist to see the latest changes.
- Forms with addy Business fields automatically become checklist-only
- These forms require user login and can only be viewed in the context of an issuance
- Public access is automatically disabled for addy Business-enabled forms
- Changes to the form require saving both the individual field changes AND the entire form
Q: Why can't I make my form public after adding addy Business fields?
A: addy Business fields contain sensitive data and require proper context. These forms must be accessed through the addy Business interface.
Q: What happens to existing responses if I add conditions to a field?
A: Existing responses remain stored but won't be visible unless the conditions are met.
Q: Can I mix regular form fields with addy Business fields?
A: Yes! You can combine both types of fields in the same form, and they will both display on the checklist.
Q: Why don't I see my latest changes in the checklist preview?
A: Remember to save the entire form before previewing. The preview pulls from the saved version, not your current edits.
Q: Can I control who sees specific addy Business fields?
A: Yes, use the permissions gear icon (⚙️) next to each field to set Read/Write permissions by role.
Q: What happens if I delete an addy Business field from my form?
A: The form field is removed, but the data in addy Business remains unchanged.
