How to submit a name change request

If you have changed your name legally, follow these instructions to update your name on file with addy.

1. Email pii@addyinvest.com with the following information:
   1. Name change document
   2. Government issued ID with photo (even if it has old name on it)
   3. Government issued ID or utility bill with updated name (if applicable)
   4. Selfie
2. Our team will review the documentation and reach out to you upon completion, or with any additional questions. 
3. Please be aware of the following terms:
   1. This change will only affect things moving forward and does not apply retroactively.
   2. Any prior investments are not updated.
   3. CSR and share certificates are static from time of issue.
